Delta Flight 1141 takes off from DFW with its flaps up

On August 31, 1988 a Boeing 727 bound for Salt Lake City lifted off from DFW without its flaps and slats set for takeoff, rolled and came down past the end of the runway. Fourteen of the 108 people aboard died. Investigators found that the crew had worked the checklist while holding unrelated conversation and that the takeoff warning system had failed to sound, which put fresh weight behind the sterile cockpit rule already on the books.
