The Centralia mine disaster and the law it forced

A coal dust explosion killed a hundred and eleven men in a mine whose inspectors had filed warning after warning about exactly that dust. The miners had written to the governor asking him to save their lives, in those words, a year before. The disaster, and the letter, forced the first federal mine safety legislation with real enforcement behind it.
