USAir Flight 427 falls out of a clear sky over Hopewell Township

A Boeing 737 on approach to Pittsburgh rolled over and hit a hillside here on 8 September 1994, killing all 132 people aboard in about 23 seconds. Investigators spent four and a half years on it, the second longest crash inquiry ever run, before concluding that the rudder had swung the wrong way when a valve in its control unit jammed. That answer also explained two earlier 737 crashes nobody had been able to solve, and the FAA ordered the rudder system redesigned on thousands of aircraft.
